Discharge coordination: how social workers use us

When a resident is cleared for discharge, the social worker or case manager calls DachiPlus dispatch — not a general booking line, but a direct facility contact. We confirm vehicle type, pickup time, destination address, and any specific resident needs (mobility devices, oxygen, companion). Typical dispatch time from the discharge call is under 30 minutes for scheduled discharges, and under two hours for same-day requests depending on availability.

At the destination — whether home, another SNF, or an outpatient rehab facility — the driver assists with the handoff and notifies facility staff of arrival. For transfers to another care facility, we coordinate directly with the receiving facility's admission team if needed.

Recurring routes for SNF residents

Many SNF residents have recurring transportation needs: dialysis three times a week, weekly oncology appointments, regular specialist visits. DachiPlus sets up standing-order authorizations through MAS for Medicaid residents and manages recurring private-pay schedules directly with the facility. Your social work team submits a standing order once — DachiPlus handles every subsequent trip without a new call for each one.

Driver continuity is built into recurring routes wherever possible. When a resident knows their driver by name, it reduces boarding anxiety and makes the trip smoother for everyone.
